Navigated liver biopsies in a closed-bore MR scanner: first clinical experience

چکیده
Introduction/Purpose Most image-guided interventions such as biopsies or tumor ablations can be readily performed under CT or ultrasound guidance. MRI may be indicated whenever a suspect lesion can not reliably be visualized with these imaging modalities [1]. In a standard 1.5T MRI scanner, however, the remaining space is practically too small to handle an instrument inside the bore. Freehand biopsies outside the bore with intermediate control imaging are usually inaccurate and time consuming. Our concept combines an optical instrument tracking with an automatic patient registration [2] where the needle is manipulated outside the bore and control imaging can be performed at any time. For interventions in a moving organ (liver), a special respiratory training was evaluated. The goal of this work was to present a more reliable solution that may be flexibly added to a closed-bore scanner environment and to report on our first clinical experience in 15 patients with liver lesions.

Preliminary clinical experience with a navigation system for biopsies in a diagnostic 1.5T closed-bore MR scanner
Introduction/Purpose Minimally-invasive MR-guided interventions are becoming increasingly important due to a number of well-known advantages of MRI over other imaging modalities. Over the last years, different concepts for interventional assistance and instrument navigation have been described for practically all regions of the body [1]. In the standard MRI environment around a closed-bore (cyl...
متن کاملPreliminary clinical experience with liver and bone biopsies guided by a robotic assistance system in a closed-bore 1.5T MR scanner
Introduction/Purpose Various concepts of guiding interventions with the help of MR imaging have been realized in the past decade. Recently, a robotic assistance system has become commercially available that allows to plan and realize the trajectories of medical instruments in a closed-bore scanner. Such a device was implemented in our conventional 1.5T MRI environment and has been clinically us...
متن کاملInitial clinical experience with a robotic assistance system for liver biopsies in a diagnostic 1.5T MR scanner
Introduction/Purpose Freehand MR-guided biopsies in a closed-bore MRI scanner are typically performed by iterative cycles of visual targeting, manual instrument feed (outside the bore) and verification of the actual position with MR imaging (inside the bore) [1]. Based on the potential needle offset seen on the MR image, the physician uses a mental representation of the 3D geometry to repositio...
متن کاملReduction of claustrophobia during magnetic resonance imaging: methods and design of the "CLAUSTRO" randomized controlled trial
BACKGROUND Magnetic resonance (MR) imaging has been described as the most important medical innovation in the last 25 years. Over 80 million MR procedures are now performed each year and on average 2.3% (95% confidence interval: 2.0 to 2.5%) of all patients scheduled for MR imaging suffer from claustrophobia. Thus, prevention of MR imaging by claustrophobia is a common problem and approximately...
متن کاملFirst clinical experience with navigated RF ablations of the liver in a closed-bore 1.5T MRI
Purpose MR imaging is becoming more important as a diagnostic tool for the liver due to its excellent soft tissue contrast. MRI is also increasingly used to control minimally invasive diagnostic or therapeutic interventions of the liver. Such interventions are often assisted by corresponding guidance systems [1]. The CT-guided placement of probes for thermal therapy and the subsequent ablation ...
